Care Management Services
Our nurse case manager collaborates with primary care providers to respond to referrals from hospitals, other healthcare entities, and self-referrals.
Community Alternatives Program
The Department of Health & Human Services' Community Alternative Program offers an alternative to placement for Medicaid recipients who need the level of care offered in nursing homes but who prefer to remain at home.
Diabetes Education Program
The Department of Health & Human Services' Diabetes Education Program offers training for people with diabetes. Services include an assessment by a registered nurse and/or registered dietitian, and up to nine hours of group education.
Fitness & Recreational Resources
Dare County offers a wide variety of Fitness and Recreational opportunities for older adults 55+ at three locations. For more information visit the center nearest you.

Health & Human Services
In addition to in-home aide services, dare home health and dare hospice, and adult protective services, the Department of Health & Human Services provides additional services for older adults.
Nutrition Services and Home Delivered Meals
In partnership with the Albemarle Commission, Dare County is able to offer older adults who are at least 60 years of age with a nutritionally balanced meal five days per week served in a congregate group setting, or delivered to the home.
Outer Banks Senior Games
The Outer Banks Senior Games is open to those 50 years of age and older who have lived in Dare or Currituck County for at least three consecutive months of the year.
Seniors’ Health Insurance Information Program
The Seniors' Health Insurance Information Program (SHIIP) was created by the North Carolina Department of Insurance to help senior citizens obtain free and unbiased counseling on Medicare health care products.
Special Assistance
The Department of Health & Human Services' Special Assistance program provides assistance to persons living in adult care homes (rest homes) or living at home who are in financial need and who are age 65 or older or who are between the ages of 18 and 65 and are disabled or blind. Individuals seeking assistance in this program area must be within the income and resource limits to qualify and meet Medicaid eligibility requirements.
Special Assistance In-Home
The Department of Health & Human Services' Special Assistance In-home program allows some individuals who could receive care in a licensed facility to remain safely in their private living arrangement with sufficient income, adequate housing, necessary health and social services, reliable supports and case management.
Tax Assistance
Assistance with filing taxes, through AARP, is also available by appointment during the tax preparation season at both the Baum and Dare County Centers.
Transportation
The Dare County Transportation System provides a daily Rural General Public demand response service that is a curb to curb, ride by appointment transportation service. This service is available to all Dare County residents and visitors for any purpose.
Thomas A. Baum Senior Center
The mission of the Thomas A Baum Senior Center is to serve Dare County residents and visitors who are 55 and older. A variety of programs and activities promote an active and healthy lifestyle providing opportunities for recreation, socialization, nutrition, and health support services.
Veterans Services
In appreciation, the Dare County Board of Commissioners provides a local Veterans Service Office to help veterans and their dependents gain information and assistance in accessing all available benefits.
